Is there a maximum dose of enoxaparin (LovenoxÒ )?
Enoxaparin (LovenoxÒ ) is a low molecular weight heparin indicated for the prevention of deep vein thrombosis (DVT) in patients undergoing abdominal surgery, hip replacement, knee replacement, and extended hip prophylaxis. It is also indicated for the treatment of DVT with and without pulmonary embolism (PE) in conjunction with warfarin therapy and prevention of ischemic complications associated with acute coronary syndromes (unstable angina/non-Q-wave myocardial infarction). For treatment of DVT with and without PE and prevention of ischemic complications associated with acute coronary syndromes, enoxaparin is dosed on a mg/kg basis. However, specific clinical data addressing the dosing of the medication in obese patients has not been reported and the product labeling of enoxaparin does not provide specific dosing recommendations for obese patients. In controlled trials, the mg/kg dosing of enoxaparin was based upon actual body weight. In the TIMI 11B trial, the weights of patients re
