Checkbox Survey Server (installed version) includes one licensed survey creators. Additional survey creator licenses may be purchased at a rate of $400 per survey creator. Checkbox Survey Server – Enterprise (installed version)includes an unlimited number of survey creators. Checkbox Survey Online (hosted version) includes one survey creator at the Basic package level, two survey creators at the Professional level, and unlimited survey creators at the Unlimited level. Additional survey creator licenses may be purchased at a rate of $400 per license per year or $40 per license per month.
