A PhD student should form a PhD committee (three members) by the end of the first quarter after completing the MA thesis (or after being admitted as a PhD student if s/he comes from a different institution). Again, the student should inform GPC and the administrator. Unlike an MA committee, a PhD committee has an official status in the graduate school.
