Is the Use of Suggested Phrases Appropriate for Employee Appraisals?

There is a controversy regarding the use of spoon-feeding managers suggested phrases when they are filling out an employee’s appraisal form. Some say that employees will realize that the words are not those of their manager and will, therefore, discount the comments. Others say they know it’s a training issue but that this is the easy way out. Apparently there are also legal ramifications if it is determined that the words are not those of the manager if an employee takes the company to court. I’m looking for actual experiences, both positive and negative in giving mangers this kind of tool.
