When sending cards directly to DXCC only the first page of the application form IS required. The record sheet is not required, but completing this form is always a good idea. Also, it is recommended that you keep a copy of this form for your records. Filling in this form electronically assures that you will have a copy. However, if you plan to take cards to a convention or an approved DXCC card checker, the record sheet is required. This is because the card checker will verify your cards against the record sheet and return the cards to you. The card checker (only) then forwards the completed paperwork to DXCC. If a card checker returns an application to an applicant to mail to DXCC, the aplication becomes invalid.
