No. Anyone can make the initial contact. Often the patient’s family or friends make the first call to us for information at 1.866.545.6964. If they are ready to request that hospice care begin, they can call 800.869.2136 or make a referral online. However, Capital Hospice must contact the patient’s physician before actual admission into the program to verify the patient’s medical elibility for hospice care.
