Is the medical history form DD 2807-2 the same as a physical?
No. This form is a medical screening form which is used to spot any potentially disqualifying medical conditions or any other medical condition that might warrant special attention. Your primary health care provider must complete this form based on your medical history. If you already have a completed DODMERB physical, we can accept that instead of the DD 2807-2.
