Is the logic model (Appendix F) required ONLY from applicants who receive funding?
Yes. The logic model is only required after the community is funded. Evaluation plan requirements differ for Capacity Building and Implementation applicants. Within 30 days of finalizing the CTIP, Implementation recipients are required to submit a CDC approved evaluation plan that meets the criteria described on pages 48–52 of the FOA (including a logic model) and is directly tied to appropriate components of the CTIP (refer to Evaluation Guide on Developing an Evaluation Plan available at http://www.cdc.gov/dhdSP/programs/nhdsp_program/evaluation_guides/evaluation_plan.htm ). The filled template example for the CBP (Appendix B) is not double-spaced and is not using Times Roman in 12-point type, so is it safe to assume that the applicant’s submitted CBP can be filled out as shown in the example— without double-spacing, but using single spacing; and without using Times Roman 12-point type, but instead using the Calibri 10-point as occurs in the example provided?
