All documentation is the same. The Medical Board requires a current doctors statement. The doctors statement should include: • a description of the illness and treatment • the doctors opinion as to whether the member is disabled from performing the job • a statement that the claimant has a terminal condition (diagnosis & prognosis) • Approved claims: • When a claim for disability retirement is approved, a letter is sent to the member advising that he/she has 120 days from the date of the letter to accept the decision. Information may not be provided over the telephone. Disability benefit estimates are also sent to the applicant. • The following forms must be submitted: For State Police • Form 24 Application for Disability Retirement • Form 4 Designation of Beneficiary if selecting basic, option 1 or 4, or Beneficiary Proof of Birth if selecting options 2, 3, 5 or 6. • Form 128 Reemployment After Retirement • Form 85 Direct Deposit/Electronic Fund Transfer Sign-up Form • Form 766 Federa
