Is the completed cancellation form all that is required to obtain cancellation benefits?
If there is an asterisk next to the field of service in section A of the cancellation form, additional documentation must be submitted along with the form. If additional documentation is required for the field of service, the borrower should call Campus Partners at (800) 334-8609 before sending in the form.
