Yes, medical documentation will be required for the birth of a child. If the Eligible Employee is eligible for FMLA leave, the FMLA Medical Certification Form must be completed by the individual’s health care provider. If the Eligible Employee is not eligible for FMLA leave, the employee will need a medical statement from the individual’s health care provider.
