It is difficult to say but you are not in the best of health anyway and I would tend to avoid surgery if possible. Any implant is a foreign material which can provoke tissue reactions and one of the common problems is capsule formation around the implants which makes them tight and hard and possibly painful. Personally I wouldn’t do it but that’s easy for me to say. I would at least talk to your rheumatologist and endocrinologist (or GP if you have moved to primary care). If you feel you really must go ahead then make sure you go to the right sort of clinic, you can check out the surgeon in the Medical Register (most large libraries) and refuse anyone who has not got specialist plastic/cosmetic surgery qualifications and be very, very sure to get the aftercare facilities specified in writing. As I said, I wouldn’t do it but it is not my body.
