Is it possible to see the questions in the eProtocol application forms before creating a protocol and to work on them offline?

In eProtocol there are four (4) choices of forms for a new application to conduct research with human subjects. As a tool to allow investigators to see the questions in the Protocol Information section (the main section of each form) and to work on addressing these questions offline, we have created the sample forms below. These documents are not complete application forms as they are missing several sections (Personnel Information, Vulnerable Subject Checklist, Study Sites, Funding, and Assurance) and they will NOT be accepted in place of an online submission. Information from the sample forms will need to be copied and pasted into the eProtocol application form.
