The “Notice of Medicare Provider Non-Coverage” form is given to the beneficiary to notify them they are being discharged from home care. The agency would also give the HHABN to notify the beneficiary that Medicare does not cover the homemaker services they are requesting; therefore, the beneficiary or another payer would be liable for payment of services.
