Is it necessary for individuals involved in the conversion to enroll in the PUNS database?
Yes, it is. For this project, however, the Division is waiving the requirement that service coordination agencies meet face-to-face with the individual, guardian, and/or family member to complete the PUNS form for initial enrollment. For this project, the form may be completed via the telephone or mail, if that is acceptable to the individual, guardian, and/or family member. In the event individuals involved in the conversion are already enrolled in the PUNS waiting list, additional action is not necessary.
