Premiums are based on income. Monthly premiums are as follows: 0-133% of the federal poverty level, no premiums; 134-150% of poverty, $4 premium per child; and 151-300% of poverty, $6 premium per child. Premium assistance is also available for families having difficulty paying premiums.
