Contact information for the person responsible for conducting the remediation and the Licensed Site Remediation Professional should be included on notification signs and/or in notification letters and fact sheets. If there is no Licensed Site Remediation Profession include contact information for the person responsible for conducting the remediation and the Office of Community Relations.
