Injections should be covered (to some extent) by Medicare and private insurance. If a patient receives BOTOX® and does not have insurance, or is under-insured and meets the criteria, or other resources to pay for it, Allergan has a BOTOX Patient Assistance Program to help. If the patient qualifies, Allergan will reimburse the physician for the BOTOX. If the physician will not waive the fee for injecting the BOTOX, the patient will have to pay that fee. For information, contact the BOTOX Reimbursement Solutions Hotline: • By phone: 1-800-530-6680, Option 4 from 9 a.m. to 8 p.m. (ET), Monday through Friday • By fax: 1-877-530-6680 from 9 a.m. to 8 p.m. (ET), Monday through Friday • Online at http://www.botoxreimbursementsolutions.
