Is a physical examination required to play an interscholastic sport?

The state of NJ mandates that you must take a physical examination and pass that physical before you are permitted to practice or play. The proper state physical examination form must be used and stamped by the attending physician. In September 2006, any student/athlete must sign the steroid policy instituted by the state of New Jersey. A parent or guardian must also sign this form. We are the first state to adopt this policy. If this steroid policy is not signed, the student/athlete will not be permitted to practice or participate. Members of the band and cheerleaders are not required to take physical exams.
