Is a local health department stamp acceptable?

No. A local health department stamp will ONLY be acceptable if the Immunization History portion of the health form was stamped prior to the 2002-2003 school year (i.e. health forms stamped during or before the 2001-2002 school year will be “grandfathered” in).
