Is a lab report needed for immunization records?

Immunization titers as well as a Student Care Center (SCC) 3-page document must be completed and/or uploaded (see below for link). Any immunizations needed upon arrival will be at the visiting students expense. If you have had mask-fit testing, you may upload it with your immunization records on VSAS or send a copy of the certificate with your paper application. Providing the certificate ahead of time saves you some steps during orientation. Alternatively, if you have not had mask-fit testing, we are able to do that for you in our Student Care Center. Please note that your application will not be considered for scheduling until it is complete with all required documents including your immunizations.
