Is a handwritten form acceptable for filing?
Yes, if you are going to hand write the form, please: 1) use black ink (if possible), 2) legibly print the information, and 3) line through the name and/or address (do not use Liquid Paper or correction tape to cover up any of the pre-printed information on the form as it makes the form unreadable for scanning purposes). Remember: After filing, these forms are available on the Internet and its important that they are readable.
