An OFA 6 Inspection Report is not being required for the vehicles being surveyed for the fleet reduction drill. However, you must complete a Std. 152 Property Survey Report and explain in the comment field that the reason for the vehicle disposition is compliance with Executive Order S-14-09. The Std. 152 is submitted to the Statewide Mobile Equipment Coordinator at the DGS Office of Fleet and Asset Management. An approved Std. 152 is then returned where a copy should maintained in the department’s file and a copy placed within the vehicle being disposed.
