The County Health Department Leadership Team should include a mix of representatives from a variety of stakeholder groups, both within and outside government. The composition of the leadership team should be driven by the goals of the program and how they will be accomplished. The proposal will be reviewed for strength of the mix of representatives aligned with the proposed work plan. It should include at least one representative from the State Department of Health and one representative from the County Department of Health. Other key members could include city and local departments of health within the county, other governmental agencies, nongovernmental organizations, businesses, health plans, or media and political leaders.
