Is a copy of the Affidavit or statement filed for a particular school available?
Yes. All Affidavits are public records. All schools filing the Affidavit should retain a copy of their Private School Affidavit for their own records. Request a copy of an Affidavit by mail at: Elementary Education Office – Private Schools California Department of Education 1430 N Street, Suite 4309 Sacramento, CA 95814-5901 Request by means of fax to: 916-319-0129; or by e-mail at privateschools@cde.ca.gov. Please include the name and address of the school. Also include the requestor’s name and telephone number, and the physical or e-mail address to which you want the copy sent. Most Affidavits filed after 2005-06 school year can be e-mailed.
