Stamped signatures are not accepted. If the doctor or health care professional used a stamp signature for this section, you [your daughter] or the person responsible for out-of-pocket costs for GARDASIL must bring the rebate form back to the doctor’s office to have this section signed with an original signature in order to be eligible for the Rebate.
