• If claim for payment is by the widow/widower, no documents are required except proof that she/he is widow/widower of deceased subscriber. • If claim for payment is by the mother on behalf of minor children of the deceased who is not a Hindu, the Guardianship Certificate issued by a Court should be enclosed. • If claim for payment is by another relative of the deceased, a Succession Certificate issued by a Court should be enclosed and also the reasons for the claim being preferred as for example, the subscriber died as bachelor/ the spouse pre-deceased ( in such cases the death certificate of the deceased spouse should also be enclosed)/etc. • The Death certificate of the deceased subscriber from the competent medical authority must be enclosed along with Form-C.
