Im trying to fill out the SGHI Baseline Health Assessment online, and it indicates that I have already taken the survey with a “Thank You” message. How can I complete my survey?
If you shared your computer and web browser with a friend, colleague or family member, you’ll need to delete and clear your cookies and cache from your browser in order to complete the survey. To delete cookies and your cache on Internet Explorer: • On the Internet Explorer 6 Tools menu, click Internet Options. The Internet Options box should open to the General tab. • On the General tab, in the Temporary Internet Files section, click the Delete Cookies button. This will delete all the files that are currently stored in your cache. Then select delete all at the very bottom. To delete cookies and your cache on Firefox: • Open Firefox and click on the “Tools” menu. • In the “Tools” menu, click on “Options”. • The Options pop-up window will open. • From along the left side of the window select the “Privacy” icon.
