If your SDB / 8(a) form is less than two years old, you may send us that form instead of completing the DBE Uniform Certification Application form. However, you will still need to complete and notarize the Affidavit of Certification that is part of the DBE Application, and the Personal Networth Statement (PNS). The PNS must be signed and notarized within the 90 day period before your Application is received by us. If your SDB / 8(a) Application is more than 2 years old, you will need to complete the full DBE Uniform Certification Application so that we have updated information. You will also need to send us the supporting documentation listed on the DBE Application Checklist, regardless of which application form you are using.
