NeuroScience is required to charge sales tax to any customer located in Wisconsin or Arizona. If our customer is charging sales tax for products sold in their facility, we do not need to charge sales tax when we ship product. Filling out the sales tax exemption form allows us to avoid that step. Please note: this number is different than your Employer’s Identification Number – EIN.
