If you die while having surgery, is this considered”accidental death” by insurance companies?

It is considered an accident if death was caused by some aspect of the surgery (surgeon error, anesthetic problem). It is not considered an accident if death is due to condition that necessitated the surgery (accident before hospitalilzation excluded).
