If the students parents are divorced or separated, what forms does the noncustodial parent need to complete?
The noncustodial parent only needs to complete the CMU Noncustodial Parent Information Form (NCPIF). On the student specific IDOC Cover Sheet (see sample of cover sheet), the student will see that CMU has an institution specific document (the NCPIF). When the student links to CMU’s form on the CSS website he/she will be instructed at the top of the form to print out both CMU’s NCPIF form and an IDOC Cover Sheet, and to send these to the noncustodial parent to complete and mail to IDOC. We do not require the noncustodial parent to complete the CSS Noncustodial PROFILE form.
