You have to submit a complete application to the graduate school of Utah State University and this application needs three references, GRE scores, TOEFL scores (for international students), original transcripts, and a statement of interest. Application forms can be downloaded from the following website: http://www.usu.edu/gradsch/ It is recommended that PhD applicants sumit at least the abstract of their MS thesis in English.
