If you filled out your graduation application, submitted it to the Office of the Registrar with the graduation fee and do not have to complete any courses over the summer or in the fall, you DO NOT need to fill out this form. The Department Approval to Attend Commencement with Outstanding Degree forms are only for students who will finish their required credits over the summer or their student teaching in the fall.
