If I enroll in an HMO for 2010, but midyear I experience a qualifying change in status and decide to change to a Motorola Health Plan such as the Health Investment Plan, will my contributions be at the regular or discounted rate? Will I be required to take the Health Risk Questionnaire?

The Health Risk Questionnaire will not be required until the next annual enrollment cycle. You’ll pay the discounted contributions rates, effective the first of the month following approval of your benefits election, provided the Benefit Election Form is received within 30 days of your qualifying change in status.
