A. Physicians with over $100,000 in Government of Ontario earnings do not need to supply any further evidence as they will be getting a full payment. However, if a physician falls below the $50,000 mark and is not getting a payment, or a physician thinks they earned over $100,000 in Government of Ontario earnings and should get the full payment rather than 50% payment, they need to send in a cover letter and provide proof of Government of Ontario sources of income. This could include pay stubs, cancelled cheques, bank statements (with personal information blacked out) for service dates between April 1, 2006 to March 31, 2007. Proof of income should clearly indicate the source of funding/income, the dates for which the funding/income is for and that payment has been made.
