If I answered ‘yes’ to one of the personal history questions on a previous Oregon EMS certification or recertification form, do I need to answer ‘yes’ again?
If something has happened since you were last certified, then you will need to check ‘yes’ and submit a written explanation. If the incident occurred prior to the last recertification period, was already reported to DHS-EMS and approved by the Department, then you do not need to report it again.
