If you have added an eligible dependent during the Open Enrollment period, you must submit supporting documentation to the Human Resources Benefits Division by 5:00 p.m. on November 20th. Be sure to indicate your 5 digit employee ID number at the top right hand side of any documentation. Please refer to the Dependent Verification information sheet for additional information.
