If I already have a version of DNS can I use it with ScreenSpeaker and if so, will it work in the same way as SpeechMaster?

Yes, provided that you have version 4 of DNS Preferred or Professional, it will work in the same way as SpeechMaster once you load ScreenSpeaker. Previous versions of DNS are not recommended but should operate satisfactorily. The benefits of version 4 over version 3 are significant; an upgrade pack is available.
