If Blue Shield offers groups the option of narrow and broad coverage, how will this be communicated to new groups?
Blue Shield of California revised the three Master Group Applications for 2-50, 51-299, and 300+ employees. On the existing Master Group Application, “Do you wish to offer coverage for domestic partner?” was changed as follows on forms with a 1/05 revision date: Blue Shield plans include coverage for state registered domestic partners only. Do you wish to select supplemental coverage to include additional domestic partners who are registered with municipalities, counties or who submit an affadavit? (check one) yes___no____ Note – Group dues/premiums will not increase for either option. Blue Shield of California has also revised many other forms to indicate spouse equivalent coverage for domestic partners (i.e., Employee Application, Refusal of Personal Coverage, and Subscriber Change Request. The revised forms are available on both the Producer Connection and Employer Connection.
