LTD ends the date the employee begins full-time military duty and is not available on a self-pay basis. Continuation of life insurance coverage for employees may be extended to the end of the 12th calendar month in which he or she begins full-time service in the military forces (land, air, or sea) of any country. Employees may reinstate their coverage at the original amount within 31 days of return to full-time employment status with completion of an enrollment form. Any increase to the life insurance amount upon the employee’s return to work will require a Life Insurance Evidence of Insurability (430 KB) form.
