Yes. The physician can submit the information using the CMS-855I form with Voluntary Deactivation of Billing Number checked. This action is necessary to ensure that the PTAN does not remain active; and therefore, susceptible to inappropriate use. The CMS-855I form is available at our Enrollment Center.
