If a government employee wants to apply for major change in name in passport, is leading newspaper cutting required to be submitted?
If Government/Public Sector/Statutory body employee wants to go for major change in name, then he/she has to submit the following additional documents: 1. Gazette Notification changing name in applicant’s department. 2. Fresh ID Certificate in changed name. 3. Deed poll/sworn affidavit as per Annexure “E”. Hence, leading newspaper cutting is not required to be submitted.
