You will need to submit a complete application for training program review, an application fee of $240 and the course materials from your training program. You may view and download the applications for first aid/CPR and preventive health and safety from the EMS Authority’s website at www.emsa.ca.gov. To access the application click on the childcare link. Note that first aid and CPR are together on one application, and preventive health and safety has a separate application. You may also view the regulations that govern these programs on this same web page. Click on the link “Regulations for Child Care Providers.” If you cannot download an application or the document that you need, you may call the EMS Authority at (916) 322-4336, extension 441, and leave your name, address, telephone number, and which application or document you want.
