OK. How did you learn that the date is wrong. From that person or organization you need to obtain a notarized documents certifying what is your correct birth date. Then you make a copy of the notarization and send the original with a letter of explanation to the city in the state where your birth was recorded (where such records are maintained in that state) and simply ask them to make the correction on your birth record, and to kindly send to you a copy of the corrected record. Also, if you can learn what they will charge you for that copy of corrected record, send that amount and a SASE also. If you don’t know what the charge will be tell them to charge your credit card or get a money order for about $25.00 and send it. If it is less than $25 they will reimburse you.

I did before. My birth certificate said I was born in 1980 when I clearly was born in 1880. but hospital records weren’t so great in my day.
