If you qualify, you can receive up to two network packages (ABC, CBS, NBC or FOX) from your choice of 5 cities, Chicago, Atlanta, Denver, Los Angeles, New York, plus the national PBS channel. In order to receive these packages, you must meet certain criteria set forth by law and submit a signed Declaration Form and other necessary documents to DISH Network. You can obtain a copy of the Declaration Form and instructions by downloading it here or by calling 1-800-333-DISH (3474).
