• The Florida Department of Health requires that school entry health exams be performed by a physician in the United States. In addition, these evaluations must be completed on the yellow DH 3040 State of Florida School Entry Health Exam form only. These forms can only be obtained from a Florida physician, Florida Department of Health, or public health clinic. The charge for this exam by MedExpress is $20. There will be an additional $10 fee if immunization records need to be transferred to the official “Blue Card”.
