Complete a Request Form and submit it by mail, fax or in person to the Office of the Faculty Registrar, 100 St. George St., Room 1006, Toronto, ON, M5S 3G3, Fax: 416-978-2487. There is fee for this service and the processing time is normally 3 business days.
