We will direct the laboratory to send the bill to your permanent home address. You can then submit the bill to your insurance company for payment consideration. Please note that Quest Diagnostics, our reference laboratory, does not accept all insurance plans.

You are required to note the laboratory in your health insurance network on the Waiver Application. If you designate Quest Diagnostics or Lab Corp as your laboratory, we will use that laboratory. If neither Quest or Lab Corp is in your health insurance network, we will give you a prescription for testing and you can take it to your assigned laboratory test station.
