If you [your son/daughter] misplaced the rebate request form, a new form must be obtained from the doctor or health care professional who administered the dose of GARDASIL. As a reminder, rebate request forms postmarked more than 90 days after the date of administration of the dose of GARDASIL are not eligible for the rebate.
